Data controller

Contact form

When you use the contact form, we collect your name, your email address, your phone number if you give it, the wing you are interested in, the membership you are considering, your message and the language of the page. Fields marked with an asterisk are required: without them, we cannot handle your request; the others are optional.

This information is used only to handle your request and reply to you. It is not used for any other purpose, and in particular not for marketing without your separate consent.

It is passed on to our technical providers, who act on our instructions: Vercel Inc. (hosting of the website and handling of the request) and Resend (sending of the message by email to Alpha Form’s mailbox). These providers are companies established in the United States; requests are processed in Ireland, but data may be processed outside Morocco, including in the United States. These transfers are subject to the formalities laid down by Articles 43 and 44 of Law No. 09-08 with the CNDP.

We keep your requests for 12 months after our last exchange, then delete them, unless a legal obligation requires us to keep them longer.

To limit abusive submissions, the server uses your IP address for no more than 10 minutes; it is not recorded.

The internet is an open network: despite the security measures in place (encrypted connection), data traveling over it may be intercepted by unauthorized third parties.

“Start” questionnaire

The “Start” journey works only in your browser. It stores locally your progress, the codes matching your answers, the result, the practice families suggested and the date, under the key alphaform:start:v1:state, so that you can resume the questionnaire. This information is never sent to Alpha Form or to any third party. It contains no identity and no medical data. The “Start over” function erases it; it is also erased automatically if you come back to this questionnaire more than six months after your last answer.

“Move” questionnaire

The “Move” journey also works only in your browser, under the key alphaform:move:v1:state. It stores the codes of your answers, the families suggested or explored, the “Practices you’ve explored” list and the date. It can read the wing (Men’s or Women’s) already chosen in “Start”, without changing it. This information is never sent. The “Erase everything” function deletes it; it is also erased automatically if you come back to this questionnaire more than six months after your last answer.

Cookies and audience measurement

The website sets no advertising or audience measurement cookies: Google Analytics and Google Tag Manager are not used. If audience measurement were introduced, your consent would be asked for beforehand and this page would be updated.

Hosting and technical logs

The website is hosted by Vercel Inc. When you visit, the host records technical logs (IP address, browser, pages requested) to ensure the operation and security of the website, for a limited period set by the host.

Your rights

In accordance with Law No. 09-08, you may access the data concerning you; ask for it to be corrected, updated, erased or blocked when it is inaccurate or its processing does not comply; object, on legitimate grounds, to its processing; and object, free of charge, to its use for marketing purposes. To exercise these rights, write to contact@alphaform.ma, by post to the address above, or come to the club’s reception desk, with proof of your identity. You may also refer the matter to the National Commission for the Control of Personal Data Protection (CNDP, www.cndp.ma).
